We exist to provide a medical home for all children, with specialty care for children in foster care, kinship care, adoptive care and developmental care in Arizona.
We serve every family in our clinic with support and connection through our most valuable gift: time with their provider. Through this connection, we become a part of the healing journey for each child.
Chrysalis Children's Clinic strives to deliver clinically excellent, restorative care to every child in our practice. Through our Restorative Care Model, every child has the opportunity to move from trauma to restoration through our child centered care connections. With each care connection, the physical, developmental, psychosocial, nutritional, spiritual and community needs of the child are supported.
Angela Cabrera, APRN, CPNP-PC is a board certified Pediatric Nurse Practitioner with 19 years in pediatric primary and specialty care and 27 years of combined registered nursing and pediatric nurse practitioner experience. She has extensive experience working with the medical and psychosocial needs of children who are in foster care, as well as personal experience as a foster parent. She has also worked extensively with children with developmental needs. Angela enjoys cooking, reading, learning, running, and family dinners around her table. She also enjoys spending time with her husband and five amazing children.

We are located in America's Health Center at 1845 E. Broadway Rd., Suite 116, Tempe, AZ 85282.
We are open Monday-Friday by appointment from 9am-4:30pm, and extended hours until 6pm on Tuesdays and Thursdays. We are available for in person appointments for all new patients. Telemedicine appointments are also available for our established patients.
After hours (urgent) appointments are seen via telemedicine for established patients.
We provide pediatric primary care pediatric services for all children, and specialty services for children in foster care, kinship care, adoptive care, and children with developmental needs, ages 0-18. This includes well exams, sick visits, urgent care needs, immunizations, advanced developmental screening, and primary care psychiatric needs. We strive to include restorative care with every visit, which includes addressing the physical, developmental, psychosocial, nutritional, spiritual , and community support needs for each child. We are also a part of an innovative collaborative care experience with Arbor Therapy, who provides speech, occupational, physical and music therapies for our children. We currently accept the following AHCCCS plans: Mercy Care , AIHP, and Banner University Family Plan. Additional AHCCCS plans and private insurance plans coming soon!

Arbor Therapy’s multidisciplinary team is comprised of highly qualified therapists that hold a high regard for patient care, specializing in all areas of developmental needs.
Arbor Therapy clinics provide a warm and welcoming atmosphere with gyms in each clinic. They offer clinic, tele therapy and home-based (limited areas) therapy. Most AHCCCS plans are accepted, along with AZ DDD and multiple carriers.

Our founder, Angela Cabrera, APRN, CPNP-PC, began her journey as a pediatric nurse practitioner in a grant funded child protection clinic with the University of Alabama at Birmingham. Over the last 19 years, she has continued working with children in the foster care system and children with developmental needs in primary care and hospital settings. Through this journey, Angela witnessed the need for more time, support, and education for children and foster families and children with developmental needs in their medical home. In 2020, the vision of a Restorative Care Medical Home was born, where every child who had experienced child abuse and neglect would receive comprehensive care from a trauma informed perspective. Her practice continues to expand to include caring for the ongoing needs of children who have been adopted and children with developmental needs.
